    INSURANCE REIMBURSEMENT – CRANIAL PROSTHESIS

    Super Bill Service and Policy

    Purpose
    We prepare the documentation you need to submit an out of network insurance claim for your cranial prosthesis so you can focus on wearing your crown, not chasing paperwork.


    Section 1. What This Is
    This is a documentation package that allows you to submit an insurance claim.
    We do not bill insurance providers directly and do not guarantee reimbursement.
    All wigs are paid in full at checkout. Any reimbursement is handled between you, your physician, and your insurance plan.


    Section 2. What You Receive
    Documents are emailed after delivery confirmation and include:

    • Itemized invoice coded with HCPCS A9282.
    • Medical necessity letter template for completion by your physician.
    • Blank CMS 1500 insurance claim form.
    • Step by step filing instructions.
    • Proof of delivery receipt

    Section 3. Cost
    Insurance documentation processing fee. $15 per order. Added after wig purchase.
    This fee is non refundable and covers administrative preparation only.

    Queen Taja Collections is an out of network provider. We do not accept insurance payments and do not guarantee coverage or reimbursement amounts.

    Client Responsibility
    Clients are responsible for submitting claims directly to their insurance provider.
    Clients are responsible for obtaining physician signatures, ICD 10 diagnosis codes, and any required prior authorizations.

    No Medical Advice
    Queen Taja Collections does not provide medical advice, diagnosis, or treatment.
    Medical sections of insurance documentation must be completed by a licensed healthcare provider.

    Processing Timeline
    Insurance documentation is prepared and emailed within twenty four hours of delivery confirmation.

    Record Retention
    We retain copies of invoices and delivery confirmation for verification purposes only.
    We do not store medical records or insurance information.

    No Guarantee of Reimbursement
    Insurance reimbursement decisions are made solely by the client’s insurance provider in accordance with plan terms.

      PROCESSING TIME
      Orders are processed within one to three business days, Tuesday through Friday, after payment confirmation.
      We are closed on Mondays. Orders placed after 5:00 pm on Friday will begin processing the following Tuesday.
      Orders cannot be changed or canceled once processing has begun.

      SHIPPING METHOD AND DELIVERY
      Estimated transit time is seven to ten business days after processing, excluding weekends and holidays.
      Delivery timelines are estimates and may be affected by weather conditions, carrier delays, customs processing, or peak shipping periods.

      TRACKING
      A tracking number is emailed once your order ships.
      Please allow up to forty eight hours for tracking updates to appear in the carrier system.

      SHIPPING COST
      Shipping is included in the listed product price unless otherwise stated at checkout.

      ADDRESS ACCURACY
      Customers are responsible for providing accurate and complete shipping information.
      Queen Taja Collections is not responsible for delays, loss, or non delivery resulting from incorrect or incomplete addresses.

      DELIVERY CONFIRMATION
      Delivery is considered complete when the carrier marks the package as delivered.
      Risk of loss transfers to the customer at that time.

      LOST OR DELAYED SHIPMENTS
      If tracking shows no movement for fifteen business days after shipment, please contact us in writing.
      We will initiate a carrier inquiry. Replacements or refunds are issued only after carrier claim approval.

      Email support@queentaja.com with your order number and clear photos of the unused unit.
      If approved, we will issue a Return Merchandise Authorization (RMA) and a prepaid return label within one (1) business day.
      Returned items must be shipped within three (3) calendar days of RMA issuance.
      Refunds are processed within five (5) business days after the unit is received and passes inspection.

      One complimentary exchange per order is permitted for size or length at the same price point within seven (7) days of delivery, provided the unit is unworn and the lace is uncut.
      Any price difference will be charged or refunded prior to shipment of the replacement unit.

      Manufacturing defects, including lace tears, abnormal shedding, or cap damage, must be reported within forty-eight (48) hours of delivery with clear photos.
      Upon verification, Queen Taja Collections will repair, replace, or refund the item at our discretion, including applicable shipping costs.

      Claims for domestic packages not received within fifteen (15) business days of shipment must be submitted in writing.

      Because all orders are custom made and shipped directly to the client, delivery dates are estimates and not guaranteed.
      Upon receipt of a timely claim, we will file a carrier inquiry and issue a replacement or refund within five (5) business days of carrier approval.

If approved, a custom invoice will be sent. Full payment is required before the order is placed. Please note.       Yes. All units are designed to be worn without glue or adhesive. Adjustable elastic bands and drawstring caps provide secure wear. Depending on your hairstyle or activity level, light hair tape may be used around the ear tabs for added security.


      Our wigs are designed for African American women, men, and gender affirming clients who want natural looking, beginner friendly solutions for hair loss or everyday wear.


      All units feature HD lace and 12A grade virgin human hair for a soft, realistic finish and long lasting wear. Lace size selection is based on personal preference and may depend on desired parting space, coverage needs, and styling preferences.


      Yes. All units come pre plucked, pre cut, and with bleached knots. No additional customization is required before wearing. As a precaution, we recommend shampooing and conditioning the unit and allowing it to air dry before first wear, especially for clients with sensitive skin or allergies. For the most seamless finish, you may also choose to lightly tint the lace to better match your complexion.


      All units start at 180 percent density for natural fullness. Optional upgrades to higher densities may be available on select styles.


      Orders process in 1 to 3 business days. Delivery typically takes 7 to 10 business days after processing. Tracking is provided once your order ships.
